EPHS Student Directory
The EPHS Student Directory is a printed directory that includes a listing of all high school students with parent/guardian names, addresses and phone numbers (unless families opt out of providing this information through the school). It is a handy item to have – especially for freshmen (much of the information stays the same over the years), and again as seniors as you are putting together plans for graduation, parties, etc.
Directories are pre-ordered for $8 each – deadline to order is September 26th. We only print as many as ordered. Two options to order and pay:
To Order Online (Two Step Process):
1. Fill out and submit the order form below.
2. Pay $8 online using the PayPal button at the bottom of the page.
To Order by Mail:
Print and fill out the order form and mail to Attn: EPHS PTO, EPHS 17185 Valley View Road, Eden Prairie, MN 55346, or drop off at the South Entrance Office, along with a check to the “EPHS PTO.”
Directories will be delivered to students in their Connections class in October.
